Gynecomastia is an enlargement of the male breast resulting in a feminine-looking chest. Most men develop breasts during puberty due to hormonal changes. This is known as physiologic gynecomastia and it mostly subsides on its own. However, for some men, gynecomastia continues into adulthood. In such cases, gynecomastia surgery may be required.
Gynecomastia can be caused by the benign growth of ductal and stromal tissue (glandular tissue) in the male breast. Other common causes of gynecomastia are obesity, the hormonal imbalance caused by hypogonadism, Cushing’s syndrome, hypothyroidism, steroid intake, hormonal medications, and alcohol/drug abuse.
Gynecomastia usually occurs when the ratio of estrogen to testosterone is disturbed. Hypogonadism causes a decrease in testosterone production, leading to an increase in estrogen. Cushing’s syndrome is caused by an excess of cortisol, which leads to obesity and gynecomastia in men. Hypothyroidism decreases the metabolism of both estrogens and testosterone, leading to an excess amount of fat being deposited in the chest area. Steroid intake, either through medications or illegal drugs, can lead to a decrease in testosterone production. Hormone therapy, such as that used to treat prostate cancer, can also cause the ratio of estrogen to testosterone to rise. Alcohol and drug abuse can lead to hypogonadism and other conditions which disrupt the hormonal balance of the body.
Gynecomastia surgery is a cosmetic procedure to remove excess breast tissue in men. This type of surgery is also called male breast reduction surgery. The procedure can be done using liposuction, excision, or a combination of both.

The surgery begins with the administration of local or general anesthesia.
If the gynecomastia is primarily caused by excess fat, Dr. Arun Jeedi will perform liposuction to remove it. A small incision is made around the areola, and a tiny tube connected to a vacuum pump is inserted beneath the skin through this incision. The tube is moved up and down several times, causing the fat to be drawn out.
If the condition is due to the excess glandular tissue, Dr. Arun makes a cut around the areola and removes it.
Sometimes the enlargement is due to both fat and gland tissue. Under these situations, Dr. Arun performs liposuction followed by excision of the gland tissue.
In extremely uncommon circumstances, the growth may necessitate skin removal as well. If contouring is required, Dr. Arun also performs the same during surgery.
The procedure takes about two hours. The surgery is completed with a chest dressing on the patient.

The patient should be able to return to work in about 3 to 5 days. The dressing will be removed after a week. Dr. Arun does not need to physically remove stitches because they are of a self-dissolving type.
The swelling will be significant for the first few days, and the surgery’s effectiveness may not be seen. But as time goes by, this gradually decreases. The scar is visible at first, but it fades over time.
Patients are advised to refrain from strenuous activity and contact sports for 3 to 4 weeks following surgery.
Like any other surgery, there are a few risks associated with gynecomastia surgery. Some of the risks include infection, bleeding, hematoma (accumulation of blood under the skin), and seroma (accumulation of fluid under the skin). These can be treated with antibiotics and compression garments.
Delayed wound healing is a commonly seen complication for smokers. If it occurs, Dr. Arun Jeedi will treat it carefully and effectively.
Other risks of gynecomastia surgery are saucer deformity and nipple retraction. A saucer deformity is when the skin heals in an inward-facing crease, which gives the chest a saucer-like appearance. Nipple retraction is when the nipple is pulled inwards and does not return to its normal position. These risks are rare and can be effectively managed and corrected by an experienced surgeon like Dr. Arun Jeedi .
